Mail your report to: Ms. Maria Manocchio, Episcopal Diocese of Western New York, 1064 Brighton Rd., Tonawanda, N.Y. 14150.
Preparation responsibility: The report is the joint responsibility of the Rector and Vestry of every parish and of the Vicar and Warden of every mission church (Canon 13, Paragraph 3).
